Equal Pay Questionnaire

The Equal Pay Questionnaire is a form that has been devised to help you to ask relevant questions of your employer so that you can find out whether you are being underpaid on the grounds of your gender.

It is not a form that you have to fill in by law.  However, it can be used as additional evidence if you go on to make a claim.

Your employer does not have to complete the form if you send it to them, but if they fail to do so this may be taken into consideration if your claim goes to tribunal.

On the form you will be able to:

• Give a brief summary of why you think you have an equal pay claim.

• Provide comparators.  These are people of the opposite sex who you believe are paid more than you but who carry out a job of equal worth.

• Ask any other questions that you think are relevant, eg: asking how pay is determined by your employer or whether they have carried out an equal pay review.

• Whether they agree that you have not been receiving equal pay.

• Whether they agree that your work is equal to your comparators’.

• To answer any of your additional queries.
